Scheduling Information
Whether you schedule your appointment by calling one of our Western New York locations, please make sure you have the following information on hand:
- Patient’s full name
- Date of birth
- Exam being ordered
- Reason for exam
- Is preauthorization required by patient’s insurance company?
- Insurance name and policy number(s)
- Ordering physician’s name
Great Lakes Medical Imaging also offers the following scheduling options for the convenience of our patients:
- Same-day availability for most locations (please note that some exams may be delayed due to preauthorization requirements)
- Insurance verification staff are available at all locations to assist you
